Supplementary Application Form

Use the Supplementary card application form to apply for an extension or supplementary card. Only first-degree relatives may be applied as supplementary cardholders. Fill out this form, signed by the Principal cardholder and the applicant, and fax to 888 6524 together with a valid ID of the supplementary card applicant.
