Instructions

  1. Print the Credit Card Application Form.
  2. Fill-out the Credit Card Application Form completely.
  3. Affix your signature on the space provided and mail to the address below together with your two (2) valid IDs.

  4. Submit additional requirements for immediate processing of your application or if you are interested for an International Credit Card. Additional Requirements:
    1. Copy of latest Income Tax Return or Certificate of Employment indicating length of stay, position, and income.
    2. Original copy of proof of residence (e.g. water bill, electric bill, or telephone bill.)

Application Qualifications

Principal applicant must have an annual income of at least P150,000.00

Classic Local: P 150,000.00 Classic International: P 250,000.00
Gold Local: P 350,000.00 Gold International: P 500,000.00

Applicant should be residing and working in the Philippines.

Note: Applications will only be processed upon submission of the application form and two (2) valid IDs.
